Upon arrival, it will be necessary to produce vaccine records, to have signed this waiver & have answered all questions to the best of your ability. These policies ensure the safety of your pet and Temple of the Dog Staff.
Behaviour
All dogs serviced at Temple of the Dog must be non-aggressive. Owners will need to verify that their dog has not harmed or shown aggression or threatening behaviour towards any person or another dog. Please remember that when your dog frequents the daycare play areas he/she will be in contact with many other types of dog and the health and safety of all animals is our main concern. These play areas will be supervised, however much like a dog park, acts of aggression can happen very quickly and without warning. If your dog does have an incident that Temple of the Dog deem unacceptable, Temple of the Dog reserves the right to remove your pet to its own separate area for safety reasons until your arrival. Owners are responsible and liable should your pet cause injuries or damages to other animals. Temple of the Dog reserves the right to refuse service and permanently expel your pet from our facility without explanation.
Health & Safety
Your pet’s medical history and information is very important to us. If your pet is not feeling well (e.g., vomiting, diarrhea, etc.), please do not bring him/her to our facility as this may affect other guests to become ill. Owners are required to verify that your pet has been in good health for the last 30 days prior to admission toTemple of the Dog. To ensure the safety and well being of your pet, owners must inform us of any allergies, intolerances, sensitivities, illnesses, physical conditions and/or physical limitations. Please note that if your pet has a communicable condition (e.g., viral infections, or bacterial infections), or is in heat, your pet must remain absent fromTemple of the Doguntil he/she has recovered.
Emergencies & Veterinarian Care
Should your pet require immediate veterinary medical attention,Temple of the Dog will transport your pet to either the nearest vet to our facility or your preferred vet (if it’s not an emergency situation), based on your direction on your registration form. Please note that as the owner, you will be responsible for all medical and veterinarian costs incurred. If emergency is due to proven groomer/handler error, not a pre-existing medical conditions, costs created by emergency will be covered by Temple of the Dog. Additional unnecessary services added to clinic visit bill (ie, vaccinations, other discovered conditions) will not be Temple of the Dog’s responsibility.
Vaccinations
Your pet must be fully up to date on vaccines and valid Vaccination Certificates or Titre Test Certificates issued by your veterinarian must be submitted as written proof. The required vaccines are Rabies, Distemper, Canine Hepatitis, Parvovirus, Parainfluenza (DHPP) and Bordetella (kennel cough). For pets with medical conditions (written proof required from your veterinarian), in which vaccinations cannot be administered, you may sign a vaccination waiver form at our facility. It is also strongly recommended that your pet is checked annually, by your veterinarian, be on a worm prevention program, and on a flea/tick/lice prevention program as well.

Grooming Instructions and Requests
:
a. I understand that the person who brings the pet(s) into Temple of the Dog, shall be the authorizing agent for all grooming instructions, if this person is the signing agent on this waiver or not. Temple of the Dog will do our best to follow such grooming instruction, if the coat & behaviour of the dog allows us to do so safely.
2.
Hair Tangles/Mats and Pet Behaviour
:
a. I understand that if my pet’s hair is lightly matted, Temple of the Dog will attempt to de-mat my pet. Temple of the Dog will attempt to contact me, if the matting condition is severe, and recommend a resolution. If de-matting is possible, an additional charge of $40 per 30 minutes will be added to the base grooming fee quoted. If the matting is severe,
de-matting will not be an option and the coat will be stripped
(also known as, pelt removal)
if groom is to continue, an additional charge of $40 per 30 minutes will be added to the grooming fee quoted. I understand that Temple of the Dog will do what is the best option for my pet’s well being and there is additional charges for the additional time and work involved.
b. If my pet’s coat is severely matted and/or neglected, Temple of the Dog can choose to shave down or strip the coat from my pet, as severe matting can cause dermatitis, skin irritation and other possible issues. I am aware that there may be underlying issues under the coat and that tight matting may cause hematomas, bruising or injuries.
c. Should my pet’s coat be severely matted, I am aware of the risks involved with shaving my pet as short as necessary to remove those mats, which could include skin irritation, minor nicks and stress. I will release Temple of the Dog from any and all responsibility resulting from stripping my matted pet. If the matting is severe, de-matting is not an option and coat needs to be stripped, an additional charge of $40 per 30min will be added to the grooming fee quoted. If this matting is so uncomfortable to remove that the dogs behaviour requires a handler to assist the groomer, and additional charge of $40 per 30 minutes will be added to the grooming fee quoted. I understand that Temple of the Dog may stop the grooming process if these things make the grooming process unsafe for my dog and I will be responsible to pay for time spent with my dog, if the procedure can not be completed.
d. I understand that if my pet’s behaviour creates difficulties in grooming, or acts in such a manner that it is necessary to use a handler, an additional charge of $40 per 30 minutes will be added to the grooming quote. If the behaviour is not improved by having a handler on site, it is possible that the dog will be need to be sent home and owner will be charged for work accomplished.
e. I understand that if my pet’s behaviour creates difficulties in grooming, or acts in such a aggressive manner that may be necessary to use a muzzle to avoid groomer injury. If the dog continues to be aggressive, Temple of the Dog reserves the right to stop the grooming process and notify the owner to pick up the pet. The owner will still be responsible to compensate for work completed on the pet. Temple of the Dog reserves the right to remove your pet to its own separate area for safety reasons until your arrival.
f. I understand that when older pets or those suffering from chronic illness are groomed or placed into a facility with other animals, they could be susceptible to an increased amount of stress. The increased stress can cause latent physical conditions (such as, but not limited to, heart, liver and kidney disorders) to become active, which could result in illness or, in extreme cases, death. As the owner or agent for your pet(s), you agree not to hold Temple of the Dog or its owners and operators, responsible for the illness or death of your pet(s) or for any expenses incurred because of the illness or death of your pet(s).
3.
Health & Safety
:
a. I understand that Temple of the Dog requires each pet to be vaccinated prior to rendering services on those pets. I must provide Temple of the Dog ,with my pet(s) current vaccinations records. The required vaccine is Rabies, but it is highly recommended that my pet be vaccinated against DHPP/FVRCP, Lepto and Bordetella. Even if my pet has been inoculated with these vaccines, I assume all risk associated with my pet’s health.
b. I understand that for my pet’s safety, it will be tethered using a non-tightening loop during the grooming process.
c. I understand that if any minor injuries occur to my pet, which are the fault of my pet, I will be responsible for its medical expenses.
4.
Late Fees and No-Show Charges
:
a. I understand that if my pet is not picked up on time, a late pick-up fee of $10/half hour may be assessed and charged.
b. I understand that if I do not attend my grooming appointment as scheduled, or if I cancel my appointment without 24 hours prior notice, a No-Show fee of $45 will be administered. My account must be in good standing prior to the rendering of any future service.
5.
Service Quality Guarantee/Medical Claims
:
a. I understand that if my pet is not picked up on time, a late pick-up fee of $20/hour may be assessed and charged.
b. I understand that if the grooming service is deemed unsatisfactory, notification must be given to Temple of the Dog within 24 hours of my pet’s grooming appointment so appropriate changes can be made. Any minor changes can be made within 48 hours of the initial groom. Temple of the Dog must be notified within 48 hours of my pet’s initial groom of any claims regarding compensation for medical attention given as a result of alleged negligence or injury by Temple of the Dog staff. A letter from a registered veterinarian may be required.
6.
Oral Care and Teeth Brushing:
a. I understand that Oral Care and Teeth Brushing services performed by/at Temple of the Dog is for cosmetic purposes only and is not intended to be a substitute for veterinarian care or advice. Temple of the Dog recommends that all pet owners, including me, make proper dental hygiene care part of their pet's regular routine and to have their pet's teeth and gums checked out by a qualified veterinarian or professionally trained canine hygienist, on an annual basis.
7.
Photography Rights:
a. I understand Temple of the Dog may take photos and/or videos of my pet from time to time during his/her stay and may post these photos onto Temple of the Dog’s website and other forms of social media.
I have read and understand the grooming policy & procedures to have my dog groomed at Temple of the Dog. I give Temple of the Dog authorization to groom and care for my dog. I understand that my dogs behaviour, and coat condition determines what work Temple of the Dog and staff are able to complete. I understand that if the coat condition is poor (matted) that Temple of the Dog will need to remove the dogs fur at a length that does not cause the dogs additional suffering. I understand that if my dog is double coated (or should not be shaved) and the coat has not been maintained, Temple of the Dog will remove the coat (shave the coat off) so that the dog no longer suffers. Temple of the Dog has the right to refuse to work on my dog for any reason they seem fit, be it aggression, illness, neglect etc. I understand that the staff at Temple of the Dog are trained professionals and will not harm my dog and will always have my dog's best interest in mind. I understand Temple of the Dog's Policies & Procedures and agree to them. I also understand that this waiver is open ended (does not expire) and includes all dogs I have ownership/guardianship of.
